Procedure for filling in the Scheda di Destinazione Lavorativa (SDL) - P002_C

The Scheda di Destinazione Lavorativa (SDL) is Sapienza's tool for recognising the risks linked to the specific job of each worker, and is necessary for the activation of health surveillance protocols.

In order for each worker to be subject to health surveillance, it is therefore necessary for the Employer/Manager to send the SDL to the competent offices, duly completed.

It is compulsory for employers to draw up the SDL for all employees, at the time of recruitment and following any change in the work assigned, as this is an important safeguard against the criminal liability imposed on them by Legislative Decree 81/08 and subsequent amendments and integrations.

 

How to fill out the SDL

The Employer has the task and the duty to provide the competent Safety Offices with the SDL for each worker pertaining to their production unit.

The Employer/Manager submits the SDL to the worker, who fills it in and signs it.

The Person in Charge and the Manager check it and submit it to the Employer who signs it and sends it to: sdl@uniroma1.it

The SDL must be completed when the job is assigned and each time there is a significant change to what is declared in it.
